6/6/2023 EnchantaVaughn Approved (AM) Client greeted peers, teacher, and QMHP by saying good morning and hello. Client was observed sitting quietly in his seat displaying positive social behaviors during morning announcements. Client appeared to be in an excited and upbeat mood as evidenced by him smiling and initiating a conversation with QMHP. Client expressed that he was feeling good and was going to have a good day. Client shared positive thoughts about his morning and continued making good choices, which were supported by QMHP and teacher. Client was observed practicing his calming and relaxation strategies by utilizing building blocks and puzzles to exercise his mind and work on his personal strengths. Client demonstrated good teamwork skills and leadership capabilities, requiring little to no help at this time. Client attended a field trip to the Children’s Museum. Client was reported on task and following teacher directives while in the community. Client displayed appropriate conduct and was able to have a good day. (PM) Client was assisted in lunch with picking healthy choices. Client often opts out of eating his lunch and is reminded of how privileged he is to have free lunch. Client was encouraged to give away his food instead of throwing it away. Client was acceptive of encouragement. Client was observed listening and sitting with other group members during lunch. Client seem to be heavily impacted by good role models and one on one support. Behaviors were maintained and managed at this time. Client displays the correct manners at the table. Client is seen conversing with peers and responding well to feedback. Client was observed following rules and being compliant of the reinforcement provided by administrative and TDT staff. Client demonstrates he is capable of sitting down for the entire lunch period without running and jumping from seat to seat. Client was able to cope and relax in his environment. (EOD) Client understands that the difference between him and the others kids are his choices. Client is observed laughing and smiling with peers and working as a team. Client returns to class to complete the rest of his assignments. Client was monitored in academic setting displaying age appropriate skills and reframing from episode. Teacher loves to see client try his best and notices that as long as he is focused he stays on task and will keep encouraging this. Client was able to achieve all objectives, as observed.
